With the evolving status of the Coronavirus (COVID-19) outbreak we want to keep you abreast of the measures being taken at the Robert Cizik Eye Clinic to protect patients and minimize the spread of the virus.

Before your visit

Check to see if your insurance plan is accepted by the Robert Cizik Eye Clinic by contacting our clinic at 713-559-5200.

What to bring

  • Insurance cards and/or Medicare card
  • Prescription glasses
  • List of current medications

On the day of your visit

Plan to be in our office for at least one to two hours. You will be asked to complete personal and medical information forms.

For Neuro-ophthalmology and Uveitis appointments, please plan to be in our office for three to four hours. We ask that you bring any necessary medical records with you at the time of your visit.

Specific information on tests and treatment plans will be discussed by you and your physician.
